For more than two decades, Johanan Rand, MD, has been helping patients rethink aging through his longevity medicine practice. Now, he’s building upon this proactive approach to health with an innovative new concierge primary care model centered on prevention, personalization, and long-term vitality.

When the fire department arrives at your home, the fire has already started, and the damage has usually already been done. It’s not a comparison most people expect to hear during a conversation about healthcare, but for Johanan Rand, MD, it’s a fitting metaphor for the philosophy that’s guided Healthy Aging Medical Centers for more than two decades. Traditional medicine, he believes, often begins after disease has already taken hold. His approach asks a different question: what if physicians could identify and address the warning signs long before patients ever become sick?

“With longevity and health optimization medicine, we’re taking a proactive approach to prevention,” Dr. Rand said. Rather than waiting for the proverbial fire to start, he focuses on identifying the earliest signs of potential health concerns, making sure the “smoke detectors are working” before serious problems appear.

This ethos has shaped nearly every aspect of Healthy Aging Medical Centers since Dr. Rand founded the practice. Over the past 20 years, he has dedicated his career to longevity, functional, and regenerative medicine, refining a care model that looks beyond treating illness to understand the broader factors that influence long-term health. While traditional medicine often centers on diagnosing and managing disease, Dr. Rand has built his practice around what he describes as health optimization, helping patients maintain a longer, fuller quality of life.

“I think traditional medicine today takes more of a reactive approach,” he said. “People go to see their doctor when they have a problem.” In his view, that model often overlooks the years during which chronic conditions quietly develop. Rather than waiting until disease presents itself, Dr. Rand believes physicians should be looking for the biological and metabolic changes that may indicate where a patient’s health is headed long before symptoms take shape.

Many of the patients who come to Healthy Aging Medical Centers, he said, arrive searching for answers after becoming frustrated with what he calls the traditional “pill-for-an-ill” approach to healthcare. Instead of focusing on a single diagnosis, Dr. Rand and his team evaluate the broader picture, including metabolic and cardiovascular biomarkers, hormone levels, nutritional status, body composition, lifestyle habits, and genetic risk factors. The goal is to develop an individualized plan that addresses not only current concerns but also potential risks that may affect a patient’s future health.

One example Dr. Rand often shares involves patients struggling with weight gain. In a traditional setting, they may be encouraged to eat fewer calories, exercise more, or improve their sleep habits. If the underlying metabolic changes remain unaddressed, however, they may return months or years later with diabetes or other chronic conditions. Instead, Healthy Aging Medical Centers identifies early biomarkers that may signal future health risks. The team then creates a personalized strategy based on the person’s unique physiology, lifestyle, and goals.

“The word proactive really means to be actively integrating approaches, getting an early start on changing the direction so that patient doesn’t develop diabetes one day,” he explained.

That same framework extends beyond a person’s lifespan. Dr. Rand frequently speaks about “health span,” the quality of the years people spend living, not simply the number of years they live. Optimizing energy, metabolism, strength, sleep, cognitive function, and overall vitality, he believes, allows patients to remain healthier and more active throughout every stage of life. For him, longevity is not measured solely by adding years, but by preserving independence and quality of life within those years.

As Healthy Aging Medical Centers continued to grow, Dr. Rand recognized another opportunity to expand his practice’s philosophy. While the center focuses on helping patients optimize their health before disease develops, many still need the guidance of a trusted physician when everyday medical concerns inevitably arise. This realization led to the introduction of Privamedis, a concierge primary care model that complements the existing practice while expanding upon its emphasis on personalized, relationship-driven care.

In many ways, Privamedis is the natural evolution of Healthy Aging Medical Centers. Patients receive the accessibility and continuity often difficult to find in traditional healthcare institutions, including direct communication with their physician, expedited appointments, and coordinated care with specialists when necessary. Instead of navigating referrals and complex healthcare systems alone, patients have a physician who understands their medical history and remains a trusted partner throughout every stage of their care.

Gary Merlino, DO, who leads the Privamedis concierge model, believes that continuity can make a meaningful difference during unexpected situations. He recalls one patient who was bitten by a dog late one evening. After reviewing photographs of the injury by phone, he arranged treatment and had medication waiting at a nearby pharmacy before it closed. In another instance, he coordinated directly with emergency physicians while one of his patients was hospitalized out of state, providing medical history and helping to guide care from hundreds of miles away. For Dr. Merlino, concierge medicine is less about convenience than ensuring patients always have a trusted, experienced physician when they need guidance most.

That emphasis on relationships echoes the philosophy Dr. Rand has cultivated throughout his career. By intentionally maintaining smaller patient panels, the practice is designed to give physicians more time with each individual, so they learn not only each patient’s medical history but also the personal goals, lifestyles, and concerns that influence long-term health. The result is a model that prioritizes continuity, communication, and comprehensive whole-body care.

For Dr. Rand, Healthy Aging Medical Centers and Privamedis are not separate ideas but two expressions of the same philosophy. One focuses on identifying health risks before disease develops, while the other provides the ongoing guidance patients need throughout every stage of their wellness journey. Together, they reflect his belief that medicine is its most effective long before illness demands attention. Rather than waiting until the fire has already started, Dr. Rand has made it his life’s work to help patients recognize the first signs of smoke, preserve their well-being, and enjoy not only longer, but healthier lives.
